Is the smart health card available only at the state and local level?

Several countries including France and Germany have national healthcare programs and issue smart card based health insurance cards to its citizens.  In the United States, there is no national program.  Patient identity cards are typically issued by your health insurance company, including the Medicare in your state, or by a hospital in your geographic region.
